Note: The wedding ceremony is shot with two cameras from two different vantage points, generally from the back of the church and from the front. We make every effort to be as inconspicuous as possible while still placing ourselves in a position to get good shots. However, our ability to get good close-ups are limited when cameras are not allowed near or on the alter or near the front of the church.

4. Pre-Wedding Preparations. We will arrive at your wedding location approximately one-hour prior to the beginning of the ceremony to set up and take sample shots. We use this time to record decorations and the bridal couple's preparation for the ceremony. Please coordinate our arrival with any preparations you want on film.

Note: We will do our best to interview everyone on camera that you have list above. However, we will not force any guest to participate if they are camera-shy. In addition, we will record the advice and congratulations from as many other guests as possible.

10. Childhood/Courtship Photomontage. Do you wish to purchase a photomontage to include in your wedding video that can also be shown at your rehearsal dinner , reception, or prior to the ceremony? A 35-picture photomontage is $100

We will set up to 35 photographs to music. Please place photos in order and lightly number the photos on the back in pencil. Place the pictures in a manila envelope and deliver them to our office at least four weeks prior to the wedding. (Shorter preparations times need approval by Video Pro.)

11. Other requests. If you have any other ideas or suggestions for your wedding video, we will try to accommodate you. After all this is your special day! Please list other requests here to be discussed with us.

12. Production Music. You may leave the music selection for your wedding video up to us. We have a large library of selections from which to choose. However, we invite your input into the music if you wish.

Note: Because commercial recordings are copyrighted, it is your responsibility to get permission from the recording company to include the songs in your video. When you provide us with the music, we assume that you have received permission from the recording companies to use the songs.

13. DVD's.

The Traditional Package comes with one DVD; the Classic with four. Additional DVDs for friends and family may be purchased at $25 each. Do you wish to purchase additional DVD's?

15. Service Agreement

We have elected to purchase the _________________________ Package from Video Pro. We understand that half the fee is due now in order to hold the wedding date. The other half is due prior to delivery of the final wedding videos.

I understand that we cannot modify or change this form after it is submit without the consent of the Video Pro staff. I also understand Video Pro will make every effort to accommodate the guidelines and wishes offered on this form; however, I understand that unforeseen circumstances such as weather or shooting restrictions imposed by the church or hall may prohibit Video Pro from fulfilling some of these requests.
